Welcome to DOES low-income self-sufficiency program supported by United Way for southeastern Michigan the purpose of this video is to give you key information on how to enroll in the LSP program the first thing you need to know is that you must have active service the applicant must be at least 18 years or older they must have an active account with DUE you must have a valid ID or driver's license as well as your actual Social Security card in addition you must be a US citizen or illegal alien for your household members we need the complete social security number and their birthdate and lastly there must be some form of income in the household when completing the application please be sure to print clearly we'd like you to include your service address as well as your mailing address if it's different from your service address that way we know where to reach you also it's important that you include your email address so that it's easy to correspond with you during the application process when submitting your identification document please be sure that the identification is valid, and it is not expired also be mindful there's the account holder the address on your identification must match the address on your DUE bill and lastly social security cards are not considered identification, but the social security card is required for the account holder when submitting documentation for minors it's okay to turn in their birth certificate health insurance card state ID card or even their school ID card when completing the application it's very important to complete all sections if you come to an area that does not apply to you, it's okay to skip over that, but please be sure to include all the information that does apply to your household this will ensure that it does not delay your processing in your LSP application it's very important that you include all 12 digits of your DT account number without it is could delay your application process the next step is proof of income the documentation you provide should show all income received for the past 30 days providing accurate budget information allows us for determined other resources you may be eligible for a few examples of acceptable forms of verification of income would be your current monthly pension statement your unemployment award letter a copy of your benefits receive from HHS your Social Security award letter the complete copy of your child support statement VA benefit letters or if you're self-employed you can provide copies of your receipts or your accounting reports applicants may be eligible for certain deductions that may assist them in qualifying for the LSP program a few examples would be child support health insurance premiums and the cost of daycare the basic needs assessment is a crucial part of the application process this assessment will allow us to provide additional resources for you and for your family before submitting your application please read over the following disclosures...
